COSHH assessment template

A COSHH assessment template gives you the headings HSE expects: the substance, its hazards, who could be harmed and how, the controls, and what to do if something goes wrong. You can download our blank template for free, or let EasyCOSHH fill it in from the product's Safety Data Sheet.

Download the free blank template

Our blank COSHH assessment template is a fillable, printable PDF with the same 12 sections as our completed assessments. It's free, there's no sign-up, and you can use it as often as you like.

Doing a whole chemical cupboard? Upload every SDS at once

With bulk SDS upload (easycoshh.co.uk/bulk/) you select all your Safety Data Sheet PDFs together, up to 50 at a time. We read each one and prepare a draft COSHH assessment per product, so you can set up your whole COSHH file in minutes. You check and finalise each draft; nothing is charged until you do.

Or get it filled in for you

Filling in a blank template means reading the whole Safety Data Sheet and turning it into plain controls. EasyCOSHH does that part: upload the SDS (or type the product name and label hazards), answer a few questions about how the product is used, and you get a completed draft to check, adapt and sign.

  • Product and supplier details, taken from the Safety Data Sheet
  • Hazard classification: signal word, GHS pictograms and H-statements (GB CLP)
  • Routes of exposure and workplace exposure limits (EH40) where the SDS gives them
  • Who could be exposed, including vulnerable groups
  • A risk rating before and after controls, using HSE's COSHH essentials banding
  • Control measures in hierarchy order, PPE, storage, spillage, first aid and disposal
  • Health surveillance and monitoring where relevant, emergency information, a review date and a sign-off box

What a good COSHH assessment covers

  • Regulation 6 of the COSHH Regulations 2002 requires a suitable and sufficient assessment of the risk to health.
  • It should reflect how the product is actually used in your workplace, not just repeat the SDS.
  • If you have five or more employees you must record the significant findings.
  • Review it regularly, and straight away if the product, the task or the people change.

Questions

Is the template really free?

Yes. The blank PDF template is free to download and use. Completed assessments are free for your first 5 (with a free account), then £2.99 each or 99p with credit.

Can I edit the completed assessment?

Yes. Every paid assessment comes as a PDF and an editable Word document, so you can adapt the wording to your workplace before signing it off.

Important

EasyCOSHH is a tool, not professional advice. You must check each assessment against the supplier's Safety Data Sheet and your actual work, adapt it and sign it off. For high-risk substances (carcinogens, asbestos, lead, respiratory sensitisers such as isocyanates) use a competent person. We are not affiliated with or endorsed by HSE.
