COSHH assessment form, completed online
Instead of a paper COSHH form, answer a few plain-English questions on your phone or computer. We pull the hazard information from the Safety Data Sheet and turn your answers into a structured COSHH assessment you can review, edit and sign.
The questions we ask
- What the product is used for and how (wiping, mopping, spraying, decanting…)
- How much is used, how often and for how long
- Who could be exposed, including young workers, new or expectant mothers and the public
- Where it's used and what ventilation there is
- What controls you already have (gloves, eye protection, training, spill kit…)
What you get
- Product and supplier details, taken from the Safety Data Sheet
- Hazard classification: signal word, GHS pictograms and H-statements (GB CLP)
- Routes of exposure and workplace exposure limits (EH40) where the SDS gives them
- Who could be exposed, including vulnerable groups
- A risk rating before and after controls, using HSE's COSHH essentials banding
- Control measures in hierarchy order, PPE, storage, spillage, first aid and disposal
- Health surveillance and monitoring where relevant, emergency information, a review date and a sign-off box
Why the Safety Data Sheet matters
Suppliers of hazardous products must give you a Safety Data Sheet. Sections 2 (hazards), 4 (first aid), 6 (spills), 7 (handling and storage), 8 (exposure limits and PPE) and 13 (disposal) are the ones a COSHH assessment relies on. If you don't have the SDS, you can still start from the product label, but you must check the assessment against the SDS when you get it.

How long does it take?
Most people finish the questions in about five minutes once they have the Safety Data Sheet to hand.
